Medical Forms
Please complete or update your medical history before your appointment using our online patient portal.
If you experience any problems accessing the portal, we recommend using Google Chrome. Alternatively, please arrive 10 minutes before your appointment so we can assist you with completing your forms at the practice.
Medical History
Please let us know about any new medical conditions, allergies or medications before your appointment.
To ensure your records remain accurate and your treatment is carried out safely, your medical history must be reviewed and updated at least every six months.
Cold Sore Policy
If you have an active cold sore, we may be unable to carry out aerosol-generating procedures such as hygiene appointments, fillings or crown preparations due to infection control requirements.
If you develop a cold sore before your appointment, please contact the practice as soon as possible so we can advise whether your appointment needs to be rearranged.
Arriving for Your Appointment
Please arrive a few minutes before your appointment time.
Appointments allow a maximum of 5 minutes´ lateness. Unfortunately, patients arriving more than five minutes late may not be seen, and the appointment may be recorded as a failed appointment.
If you are attending with children or family members, we recommend allowing extra time to ensure you arrive promptly.
Cancellation Policy
We require a minimum of 48 hours´ notice to cancel or reschedule an appointment.
If less than 48 hours´ notice is provided, or you fail to attend your appointment, this may result in missed appointment charges, loss of any deposit paid, or may affect your ability to continue receiving NHS treatment at the practice.
Patients on a dental membership plan may choose either to:
• Pay the applicable failure to attend charge and retain their annual appointment allowance; or
• Have the missed appointment deducted from their annual plan entitlement.
Missed Appointment Charges (Private & Plan Patients)
Appointments cancelled with less than 48 hours´ notice, missed appointments, or appointments where you arrive too late to be seen may incur the following charges:
• £1 per minute for examination and hygiene appointments.
• £3 per minute for all other treatment appointments.
Outstanding charges, together with any required deposit, must be paid before a further appointment can be booked.
Repeated missed appointments may result in the practice being unable to offer future appointments.
Patients who choose not to pay the failure to attend charge will have the missed appointment deducted from their annual dental plan allowance.
NHS Appointments
Missed appointments or cancellations made with less than 48 hours´ notice may affect your ability to continue receiving NHS care at the practice.
Following two failed appointments, we may be unable to offer further NHS appointments in accordance with our practice policy.
Booking Online
Private new patient examinations, routine examinations and hygiene appointments can be booked online at your convenience via our online booking system.
Deposits
A deposit is required to secure all private treatment appointments.
New patients will normally be asked to pay the full appointment fee at the time of booking.
Keeping Your Details Up to Date
Please let us know if your address, telephone number or email address changes so we can keep your records accurate and contact you about your appointments when needed.
Appointment Reminders
We send courtesy appointment reminders by text message and/or email whenever possible.
However, these reminders are not guaranteed, and patients remain responsible for attending their scheduled appointments or notifying us if they need to cancel or rearrange.
Payments
Payment is due in accordance with the practice´s payment terms.
Outstanding balances, including missed appointment charges, must be settled before any further appointments can be booked.
Where payment remains outstanding and reasonable attempts to recover the balance have been unsuccessful, we reserve the right to refer the debt to a third-party debt recovery agency. Any reasonable recovery costs incurred may be added to the outstanding balance where permitted by law.
